They can happen during OLCI or at airport kiosk. Also AF/KL offer DoD upgrades to all. The price carries depending on paid fare class. KL can also offer miles+copay upgrades DoD. Again amount carries based on paid fare class. For DoD miles+copay upgrades KL can even use DL Skymiles (at least at AMS).
