CkCrunch's solution will certainly work, but I don't think it's necessary (unless one of the myriad recent rule changes has affected things). I've definitely had unused and unrebooked award tickets from so many months ago that I didn't even know the PNR or exact flight, but was still able to work with the res agent to find it and rebook it.
