I am in the process of applying and wonder if someone can help with the following queries:

Work address.
I am a director of my own Limited Company. My home is my office. Should I use this address or my registed address which is my accountants?

Travel History
During a trip to France I made a day trip to Monaco and Switzerland. I did not enter either using my passport. Should I declare these as visited countries?

Also, I transitted Amsterdam enroute to Italy - should I declare I have visited the Netherlands?
