I stayed in the Four Seasons Chiang Mai five years ago and it was excellent then. I recommend the pool villa.
Four Seasons Koh Samui I was at ten years ago so couldn't give accurate advice [MENTION=143771]chinmoylad[/MENTION] was there recently.

For Bangkok, Banyan Tree is lovely and great value for money.
