Call +44 20 3684 3774. Say "Avios Booking" at the prompt. Say your IB number and passport number to authenticate your account.

You will be put through to a representative who can assist with what you're looking for. Make sure that they also request the refund of the Avios at the same time as the cancellation. Takes about 10 minutes.
