If you purchase a subscription directly through the Priority Pass website, there are 3 different subscription tiers. Some of them are limited to a certain number of visits, and some are less restrictive (I don't recall the plans off the top of my head). The information is all available at the priority pass website though. And yes, any limitations on number of visits would be annually.
If you get your subscription through Amex (or other credit card), most of them are unlimited visits.
