I wouldn't consider them true "luxury" hotels but some of the better options I found when I researched this were:

WHITE
Hotel in Vilnius | Hotel Pacai, a Member of Design Hotels?
Terra Nostra Garden Hotel | Furnas, Sao Miguel, Azores
