Make sure you will be using a
9-digit code (and not one received via email).
And read carefully the wiki's FAQ 3, which says, among other things:
Caution:
12-digit mailers may allow the name change only to deny the application.
E-mailed mailers do not circumvent the 24-month rules even though the application page does not have 24-month language.
