Pretty sure the change fee is per change per ticket. Typically on a domestic itinerary booked at once, PNR, booking, and ticket are the same thing, but not always. So if you make all the changes at once and it's one ticket, one change fee. If you make changes at multiple times and/or there are multiple tickets, multiple change fees.
