I just want to add that I had to contact Accor Plus about something unrelated, so I asked them about this, and got the following reply:

"When you are booking the same rate type for every day and that rate type says it includes breakfast daily, it should be reflected in the booking and be honoured by the hotel."
"Should you make another reservation which offers breakfast daily where your reservation meets the conditions but they do not display correctly on your confirmation, please do not hesitate to screenshot the offer conditions, and send this along with your booking confirmation in an email to the hotel and include Accor Plus on [email protected] so if the hotel replies in the negative, we will be pleased to intervene to clarify to them the offer conditions."

Yes, it is an issue that shouldn't be happening, and is an email that shouldn't need be sent, but at least it should (hopefully) be addressed prior to arrival at the hotel.
Of course, that email address above is only for members of Accor Plus - if you're not an Accor Plus member, presumably you'd need to CC in Le Club.
