Follow-up -- I checked my Amex statement this morning, and the refund has already posted. So 45 days seems like a very worst case scenario... it was 24 hours this time around.

No harm done, but a bit of a shame about the all-inclusive blurb being buried in T&Cs.

Shame - would have been half way toward spend qualification for those points... oh well, at least I have an excuse for some more travel!
