The payment is made by bank transfer, how would you prefer to communicate your account details? So long as you are using a BA form on a BA website then I think you're fine, the Certificate needs to be shown as issued by GlobalSign (click on the padlock next to the URL). Otherwise I guess you could leave it blank and ring the information in, but I suspect that will be a more clunky route.
