The fees are charged each time the ticket is reissued (if you change both legs at the same time the fee will still be EUR214).
If you cancel you will get back the Net paid fare minus the cancellation fee. In addition you will also get back the full taxes and fuel surcharges. Copied an example of a cancellation confirmation from QR (outbound business promo [flown], inbound economy promo [cancelled]):
A general comment: I would only book economy promo if I know I'd use the ticket. The high cancellation and change fees (plus that the price is likely higher at reissuing date) makes any changes to the itinerary a very bad deal.
