Surely, it's an insult to the waiter anywhere. It's just that in some places, such as America, the rich-poor, master-servant dichotomy is so pronounced that wait staff swallow the insult and grovel for that gratuity out of desperation. Or demand it, if they're feisty enough. Tragic and pitiable situation, altogether. One of the worst aspects of visiting the USA.
