roadsleeper. If the maximum time of six months is valid for all missing mileage requests (online or otherwise), then your mileage is probably lost.
Ticket number is requested on the online form but not required.
I don't know if it will work for me either as I'm crediting my miles to AAdvantage and not Asiamiles. I called MPC and they did a manifest search and said that their computer showed that I was on that flight. But they can't pass the information to AAdvantage.
I called AAdvantage and all they wanted was a confirmation in the form of a itinerary that showed that I was on the flight.
When I checked in at NRT, I thought something was amiss when my boarding pass printed out my AA number but without status. I asked the check-in agent to re-enter my information and after a few tries, it did. Maybe that had something to do with it. BUt it didn't make sense that one sector got credited and another on the SAME day didn't. I'm banking on getting around 15K miles for that sector alone. So I hope I get it soon!
