"Mr. Charabin: On Monday, he was searching for the phone conversation and looking for any more details the captain gave dispatch. Normally the crew sends an ACARS or calls the controlling dispatcher. The event seemed innocuous. Typically, pilots tell dispatch after the flight once the doors are closed. He could not find any ACARS messages from the crew.
He classified the report as green as there was inherent danger but it was not fully defined. It could have led to danger for the passengers. It would get elevated to yellow if there was a passenger injury such as abrupt movement causing passenger injury. A red report is a loss of aircraft. A white report is a nice to know reportable event. Green is the most common category.
If he had known they had lined up with the taxiway, he would have elevated the category to yellow. He would have had to tell management and there would have been an automatic call between the crew and the duty pilot, so that the duty pilot could get the story straight. The decision to pull the CVR is a subjective call. He decides with the duty pilot to tell maintenance to pull the CVR. This is a decision they would always discuss with management of flight ops and corporate safety."
