Three weeks after receiving my AA card, I got hit by Citi's SSA 89. All purchases were for normal daily spending. When you are asked to fill out the form, you must wait for it to come thru USPS. They will not email you the letter and the form! I followed the instruction and mailed in the cover letter itself, page 1 of SSA 89, and a copy of my SSN card. Called 5 days later to verify the status. I was told they need page 2 as well. BUT, page 2 is information only!!! It doesn't even have any spot for signature nor require me to fill out anything. Now I will have to wait for the same letter to come and do it again. I asked why page 2 is also needed. Citi fraud department rep said "We want to have it for the documentation purpose." If so, Citi should have specifically asked me to mail in both pages instead of "The completed and signed Form SSA-89." Be aware!!
