The agents pointed me to fill a blue arrival/departure form, different than the yellow form that United gave me. I asked the United flight attendants if the form they gave me is used for TWOV and their answer was yes, which was clearly false. Don't bother filling the form they give you during flight if you are going for the TWOV.
I'm not even sure airlines would know about the blue forms... they'd have no reason too as the crew would never see them because they have special expedited crew lanes.
The yellow form was the standard visa and TWOV form until sometime in 2017.
