Just a data point: When I initially applied for my CSR, the online offer for 100K bonus miles had expired, but it was still available in-branch. So I went to my local Chase branch and applied there. I'm not sure what they did, but they got the new card overnighted to me. Still took three days, as apparently the card manufacturing could not be sped up, only the shipping, but it arrived FedEx overnight.
