I did a quick search at www.travelocity.com "flights", "flight listings" (lower, right-hand on that page), enter MKE and BKK and Oct 19 and UA,voila, no published H fare for these city pairs.

I bet there are published H fares ex SFO, can you ask if you can route through there, get an H fare for those segments, and even get upgrades. I confess that I do not know if this is possible.

Not sure why UA does not have H fares published for these city pairs? Not sure if they will at some time in the future?
