Does anyone have experience with this particular card? Normally this probably belongs in the credit card part of the forum, but I'm interested specifically in the EVA co-branded card. The benefits seem pretty good for people who fly EVA often (in addition to the usual suite of Platinum benefits).

There doesn't seem to be an application form on the site (whereas the regular Platinum does, on the Taiwan website), nor is the annual fee listed.
