"Travel assistance Request" sounds like that during the booking of the ticket (maybe through a TA), assistance was requested. For example, when the person is elderly and might be mentally confused to make it on her/his own way through the airport.
I requested assistance for my grandma before and upon checkin there was no such card issued and in the end my grandma did not actually received assistance upon arrival so I am really curious what is stated in this card.
