The only time I have had BA flights not post was when my TA entered my QF FF number without the leading zeros. The BA system accepted the number as a QF FF number but the BP showed no status (should have been OneWorld Emerald) and points did not post - except for the flights after the BA F lounge agent found the problem and fixed it.
