I alway ask for an extra bottle so I don't have to ask for one in the middle of the night...as for the pillows and bed, neither have I, plus I actually prefer doing it myself. On a few occasions when the cabin was full, I just offer to do it myself because i really don't want to wait for the crew to finish the other seats.
