Kaiser Permanente now have chip, but their external contactless readers are still disabled. They also capture signatures both on-screen and with paper.

On another note, I went to Lush yesterday and successfully used Visa Credit via Apple Pay (Debit MC errored out last time). It processed as EMV, with “No Signature Required” included in the transaction record (they actually print a full terminal transaction record on the receipt!).
I wonder if CDCVM was used. They use Ingenico iPP terminals, and I always see “No Signature Required” on Ingenico receipts when CDCVM is used (which I believe can be confirmed since Amex Apple Pay does require signature at such a merchant).
