Turns out my emergency contact details were not entered. After setting the field "declined" to yes all bp printed. The Lufthansa agent commented this must be custom to SAS as she had not seen this field in altea before.
SAS is the only airline I have come across that requires even the paper form to be filled put if traveling on a USA passport. They say it is a US requierment but then they seem to be the only ones to know about it
