I can't think of an inherent reason why it wouldn't work. They should certainly be able to book an EXST on a separate PNR and cross-reference it to the main passenger (I had DL do this very recently, FWIW). I don't see why that seat couldn't be purchased with miles, B6 allows you to redeem miles for other passengers, correct?
