If you have a smart phone, good idea to have the United App on it. You then have access to a "digital" card on your phone, which can be used for lounge entry and other uses.

It is variable. Most of the time, boarding pass only works. Some times, a card is required....as I personally encountered at MUC in Sept. 2017.
