Each airline has its own discretion. If you have a compelling reason with proof, an airline can do from nothing to a full refund.
Again - it depends on how you deal with the airline.

The only issue, as I see it, will be the crutches may be too long for overhead, that the FAs need to find storage for you.

In most of the case, yes.
Rather than thinking a new program, simply giving up may be easier. Even you may be able to accrue, you may not even be able to use it after all.

No. It is not, unless it is issued as a second passport.
