(Just give your name as the business name, your address as the business address, your phone as the business phone, sole proprietorship, 0 years in business, $0 business income, etc,) Is there some reason why you can't get business cards?

The above approach has worked for me at Amex, BofA, Chase, and Citi. The only place it didn't work was obscure FNBO.
