Now he is determined to fly anyway because, well... you know how older people get when it comes to money, especially $500+..

I guess he should have purchased some kind of travel insurance, but he didn't(again, you know how older people are)
any advice appreciated...
If I knew my seatmate had a flu and was inconsiderate I would write down his/her seat number and report it to the CDC and ask for compensation from United for getting sick and from them having a lack of duty of care. Why do people not cover their mouths when they are sick and go out in public? The least your relative can do is wear a surgical mask and use hand sanitizer, travel with Clorox Wipes and cover their mouths when they cough, take Tamaflu.
If a Doctor would write a letter United and other airlines should refund or offer a Travel Voucher for people who are considerate to tell the airline that they are sick and provide evidence of the illness.
