In my experience, if there is a "gap" in the flow of passengers, then you can step from your seat into the aisle and fill it. Otherwise, you let people already in the aisle keep going. Since not everyone gets up at once, there are usually several gaps you can use to get in.

This seems to be fairly efficient to me. I doubt any other system could improve the process by more than a few seconds, which seems completely immaterial.
