The confusion here on FT about what happened and when likely happened at UA. Submit a request with all of the correct references and with clear & concise language. It needs to be really precise and, for that reason, I would have a friend carefully review it and resolve any questions the friend may have.

If this does not result in satisfaction, file a DOT complaint.

With that said, a few useful notes for others:

1. Don't mess with the tickets, fares, refunds and the like until the ticket is completed. If you are not somewhere where you can and are willing to check things carefully, just hold off. In addition, in this situation, E+ might have opened up and you would might have grabbed the seats.

2. When things go wrong, whether through bankruptcy or other, don't count on others to fix them. The second that Island Air stopped operating, was the time to call UA. If nothing else perhaps there were E+ seats at that point/
