Was the refund request verbally done or did you write an email?
The phrasing you used to request the "refund" is critical.
Did you ask for an E+ access fee refund?
Or did you ask for a ticket refund?
Or what?
A point to add to the confusion is UA was freely providing tickets refunds due to the Island Air situation.
