Not quite clear, but I'll guess UA re-booked you KOA-mainland instead of KOA-HNL-mainland and maybe only regular economy seats were left out of Kona? Sounds like miscommunication could have been easily possible. Are you saying UA refunded the ENTIRE round-trip cost? You can't book an economy plus ticket anywhere - no such thing exists. You bought an economy ticket and then bought the E+ seating. These are separate events and do not add up to an "economy plus" ticket. If you buy an economy ticket and a burger inflight, that does not make it a economy burger ticket. Just pointing out that E+ seating and burgers are just add-ons beyond what the actual ticket is (ie the basic cost of the flight). I think you could have easily miscommunicated your desire to UA.
