My concern is the fact that in the multi-city booking option when adding the flights I can't search for flights YVR-BCN but need to search separately for YVR-LHR and LHR-BCN (same issue on return from Milan). I just want to make sure I'd be protected at the Heathrow connections (makes sense that I wouldn't be in between BCN and Milan).

Yes, I'm sure that's how it works. I've "forced" itineraries using Multi-City tool a few times - they always come out as a single ticket with a single passenger reference and I fully expect you get all the protections that go with that.
There are probably far more experienced flyers than me who could give you comfort where things have actually gone a bit wrong but I'd be totally comfortable with what you are looking at.
