It seems the message differs depending on whether you are seeing it while filing a new claim (in which case it does NOT include the "for certain claims" language) versus on an existing claim (in which case the "for certain claims" text is added).

That text got me thinking that perhaps they are applying some logic to the process, so I did a little experimenting. It appears that the threshold for this requirement is > $5. If you are creating a claim for $5.01 or more than the attestation form is required. For $5 and under, there is not this requirement.

I have shared feedback on this with Chase and encourage others to please do the same. It seems like a higher threshold would make a lot more sense: maybe $20 or $50.
