Based on OP's LY status I guessed that it was a fare from Israel, and it looks like the change fee matches most flexible fares from Israel. Below is the full text of the Section 31 rules for (as an example) ULRFILW from TLV to SFO on January 15th. As I suspected, some of the conditions allow for free changes while others require a change fee (emphasis mine) ... but have fun parsing all the boolean logic:

I didn't spend too much time with it, but basically it looks like there is no change fee as long as there is no change to the first flight. (All of the conditions for "Before Departure" with no charge require no change to the first flight coupon, and the "After Departure" section implicitly assumes that at least one segment is flown.)
