Like so many things, it's not foolproof. The bag could have got attached to one going to Verona, human error, technical error.
Don't see that it is a significant security breach. The bag will have been screened and bags do regularly travel without their owners (missed connections etc). The risk is where passengers are able to contrive to create this situation. This didn't happen here and even if it did, the bag would have still been appropriately screened.
