I discovered a 3rd way recently, when booking flights over the 'phone with BA. I was offered a car rental as well, which I usually book separately with Avis/BA. As the agent quoted some £50 less than Avis/BA were quoting, I went for it. I then got a completely different voucher from the usual one, by e-mail from Avis. The only thing missing is my Avis Preferred number, which I will get added later.
The rest of this novel document also says:

However, for your peace of mind Avis offers you the option to purchase additional insurances locally when you collect your car that either waive or reduce the amount of this excess.In most countries your car will be supplied with a full tank of fuel. If you return the car with a full tank you will avoid any refuelling service charges but do check this when you collect your car.
DON'T FORGET to bring this document along with you when you pick up your car and remember your payment card(s) and driving licence too - if it's a new style UK licence you only need the photo card part. When renting abroad, please also bring the passport or identity card of main driver and any additional drivers.
I hope the "Mandatory Charges" [the usual Facility Fee etc, plus taxes] won't be added on later, thus negating the £50 implied saving
