IMO, follow all the rules, keep copies of everything, and submit early. Then when you don't get a reasonably timely response, call and demand a manager. If you've done everything correctly, the manager can get things approved. Just don't cry wolf unless if you've got your own ducks lined up first.
I had to threaten them today. I have a Price Protection claim that is now going on TWO FULL months after I got the "Final decision within 5 days" line. Unbelievable.
