Later, I saw that I was on the upgrade list, but my wife was not, even though we were on the same PNR. At the SkyClub, a helpful agent said that this, too, was a recurring issue--in this situation, each passenger needed to be added manually. When she added my wife to the upgrade list, we were immediately linked on the list (with her as a Silver immediately behind me, a Plat). So the system knew we were on a single PNR, but had somehow separated us.
So two issues: not being automatically added to the upgrade list on the second leg of a trip; and, to correct the omission, both passengers needing to be added individually to the upgrade list.
I don't know how widespread these issues are, but I'll have to check things more carefully in the future.
(I seem to recall a version of this issue being raised before on FT, but my searching has been unable to locate a pertinent thread. Sorry if I overlooked it.)
This is a recurring issue with the +1 UGs and it's annoying. Before my window, I always make sure the UG is checked under manage UGs when traveling with a +1. I've actually never heard of it not showing up on only one segment though (typically it's all segments)
