We're unlabe to approve you at this time & you'll recive an explaination in mail. I suspect the denial was due to a different last name?
I'm not sure. But I doubt that that is the reason. It seems that if you can change the first name and get approved, you should be able to change the last name as well.
The wiki says
after you authenticate the offer code and are redirected to the application page, you may enter a completely different applicant name.
Please report back what reason they give you for the denial.
