Don't cancel your stay in Punta Cana, most hotels in the Puerto Plata area are not up to the level of the Bavaro Palace. The trip to the 27 waterfalls is indeed great, but not worth the change. The beach in Punta Cana is much nicer, and the area is, in my opinion as a local, much safer than Puerto Plata.

My recommendation is to either to hire a private driver (but unfortunately cannot recommend any, as I drive in the DR all the time) or keep searching, I just found this tour operator that seems to organize day trips from Punta Cana, maybe you can give them a try:

Short trip to Haiti... Long and complicated. You can either fly (45 minutes flight from Santo Domingo) or take the bus (6-7 hours each way from Santo Domingo, so 9-10 hours from Punta Cana). IMHO, 7 days in the DR will not be enough to do the activities you want to do and also travel to Haiti.
