Copies of a card and statement must be your own, of status you have with another airline. (Using a copy of credentials from anyone else and modifying the name would be fraudulent.)

I suggest using your cell phone camera to "scan" any physical documents you need to - it's an easy method and the quality is usually good enough for airline purposes.
