It does seem to be a pretty common issue! I'm actually surprised I never ran into it with my maiden name. I've had many passports over the years in my maiden name and they never added the space. I've also never had anyone else add it in either.

It appears that many government computer systems simply cannot handle having a cap placed in between lower case letters. Some data entry people seem to have created their own solution by inserting a space after the lower case "c"! You either get the cap, prefixed by a space or the required cap remains lower case!
I thought maybe this was the reason they added the space on my passport, but the application was in all caps! Frustrating!
