When I've got that, it's because the flights were on different ticket numbers.

E.g. flights 1-2-3-4 were on ticket 125-XXXXXXXX01
Flights 5-6 were on ticket 125-XXXXXXXX02

They had to be entered separately.

Same if flights were changed after departure resulting in a new e-ticket for the remaining flights.
