What's the official rule is one thing.
What's enforced is another.
And what's ethical or 'good practice' is something else.
Often people such as gate and check-in agents (and, well, humans in general) will discard the rule to favor the path of least resistance. It's more emotionally costly and awkward to have to reject people - and especially people tied to favored guests.
That's why in practice check-in companions don't get turned away at the desk.
Supposing the rule actually IS that premium guests have +1 privilege. Should that rule be honored for a premium guest + 1 friend , but not for a premium guest + spouse + child ?
So back to some common sense..... or

* If a premium flyer is with a colleague without access, I dont mind if they tag along even if its not
in the rule
* If a premium flyer spots 5 acquaintances in the neighboring line, by total serendipity, it is totally not cool for them to be
guested in.
