I never wait 4 weeks because I will forget. As long as I know the miles may not be properly credited, I will start the process.
In any case, the crediting of miles is quite fast (within 1-2 days of the flight) if the membership number is properly recorded, so to make us wait 4 weeks is unnecessary, imo. The missing mileage form is also very outdated, look at the United logo in the sample. :P
Online Submission - Log into Krisflyer > Miles > Claim Missing Miles. You can put in an online submission with the necessary details.
