Imho with Chase sometimes it's hard to pick out one item for closure.
#3 80% in 5X's points could be a problem?
#4 Having a card closed can be a red flag but sometimes it's nothing.
#7 With Chase again it varies. They are known to do soft pulls to see if you are opening cards even if you're not applying for more Chase cards.
#8 MO's into a Chase account is a bad idea. If it was 10K a month that could bring a review. $120K a year is high. Also it shows MS. May require a report to the Federal Gov.?

With Chase it's hard to pin point why they do a shut down. In your case it might be a combination of things. Did something happen that they would look at every thing? Usually a shut down like yours happens after a internal review.
